This is the single document at the center of flying with a Service Dog. The Department of Transportation created it in the 2020 final rule. The form asks you to attest to three plain things. That your dog is trained to do work or tasks for a disability. That it is in good health. That signed attestation is what the airline files for your trip. No outside registry, no purchased certificate and no database membership is part of the federal rule.
There is a second, shorter form for one situation only. If a single flight segment runs eight hours or longer, an airline may also ask you to attest that your dog can either wait the whole flight or relieve itself in a sanitary way. Most domestic trips never touch this form. Save your energy for the main one and only add the long-haul form when an eight-hour segment is actually on your itinerary.

An airline is allowed to ask for the DOT form up to 48 hours before your flight. That window is the rhythm of the whole process. Book your seat, find the DOT form on the carrier's service animal page, fill it out and send it back through the channel the airline gives you, well before that 48-hour line. Doing it the day you book is the calmest way to fly.
Booked a last-minute trip inside the 48-hour window? You are not stuck. The same rule lets you present the completed form in person at the airport when the flight was booked too close to departure to send it ahead. Bring it on paper and on your phone, hand it to the agent and keep moving. Early is better, but late is still allowed when the booking is fresh.

A word on Support Animals, because the change catches people out. Since the 2020 rule took effect in early 2021, US airlines are not required to treat a Support Animal as a service animal in the cabin. A Support Animal comforts by its presence. A Service Dog is trained to perform tasks. Only the trained dog now carries the cabin right. An airline may still accept a Support Animal, but as a pet, which can mean a fee, a carrier and size and count limits. Plan that one around the airline pet policy, not the Air Carrier Access Act.

Your dog rides at your feet, inside your own foot space, not in a paid seat and not blocking the aisle or a neighbor. The same rule that gives that access also holds the dog to a standard. A Service Dog must stay under your control and behave the way a trained working dog behaves. No growling, no lunging, no jumping on people, no barking that will not stop and no relieving itself in the cabin. A dog that does these things can be treated as a pet or refused, even a real Service Dog.
Crew may ask you only two things to confirm the dog. Whether it is needed for a disability, and what task it is trained to do. They cannot ask about your diagnosis and they cannot make the dog perform on demand. If your dog is large, the foot-space rule still applies, so call the airline accessibility desk ahead about a bulkhead row or buying a second seat. A heads-up before travel day beats a surprise at the gate every time.
